In exercise of the powers conferred by sub-section (1) of Section 24 read with sub-section (2) of Section 14 of the Madhya Pradesh Vidyut Pradaya Upakram (Arjan) Adhiniyam, 1974 (No. 38 of 1974), the State Government hereby makes the following rules, namely :-

Short title.- These rules may be called the Madhya Pradesh Vidyut Pradaya Upakram (Arjan) Dawe Niyam, 1980.
Definitions.- In these rules, unless the context otherwise requires,-

(a) "Act" means the Madhya Pradesh Vidyut Pradaya Upakram (Arjan) Adhiniyam, 1974 (No. 38 of 1974);

(b) "Section" means a section of the Act.

Form of application for claim.- A claim under sub-section (1) of Section 14 shall be prepared by presenting an application in the form appended to these rules type written or printed on one side of foolscape preferably water mark paper in duplicate.

Form of application for preferring claims to the Claims Officer under sub-section (1) of Section 14 of the Madhya Pradesh Vidyut Pradaya Upakram (Arjan) Adhiniyam, 1974.

Before the Claims Officer

Name and address of claimant (in case the claimant is an Authority/Company/Corporate Body constituted under the provisions of any Act the reference to the said Act may please be quoted).
Name of the electrical undertaking (whether the Jabalpur Electric Supply Undertaking; the Saugor Electric Supply Undertaking or the Dewas Senior Electric Supply Undertaking) to which the claim of the claimant relates.
Name, address of the Licensee [whether the Jabalpur Electric Supply Company Limited, the Saugor Electric Supply Company Limited or the Dewas Senior Electric Supply Company (Pvt.) Ltd.] against whom the claim is made.
Statement in concise form of material facts constituting the cause of action and when it arose.
In case the claim was barred by limitation on the 10th September, 1974 under the law of limitation, the ground upon which exemption from such law is claimed.
Other particulars which the claimant may deem fit to state in support of his claim.
List of documents on which the claimant relies in support of his claim (True copies of such of the documents as are in possession or power of the claimant be annexed).
Relief claimed (the amount of claim be mentioned both in figures and words).
